These include: the immediate introduction of a nationwide pre-school education service for all children with autism; the extension of the school year through the month of July, in the case of all special facilities catering for children with autism; a doubling of the level of special needs assistant cover provided to each special class catering for children with autism; the appointment of an internationally recognised expert in the field of autism to advise my Department on the development of services in this area and to liaise with the special task force on autism which I recently established and which is due to report to me within three months; the appointment of a clinical psychologist to assist my Department in the process of identifying individual children with autism for whom a special response is required. I also establishing an inter-departmental team of high level officials involving my Department and the Department of Health and Children to co-ordinate our response to the needs of persons with autism. I confident that this arrangement will further strengthen the close coordination which already exists between my Department and the Department of Health and children and its agents in this area. My Department is also in ongoing contact with the Department of finance in relation to developments in the special needs area. Indeed, I wish to acknowledge the Department's positive response in this area as reflected in the very significant growth in the level of additional resources which have been devoted to the special education area in recent years. It is my intention to ensure that parents of children with special needs are facilitated in having ready access to all the information they require. Abstract Hyalohyphomycoses caused by Pseudallescheria boydii and Scedosporium apiospermum have recently been on the increase. To find the appropriate treatment for this emerging disease, we examined the antifungal susceptibility of 10 isolates of P. boydii and 17 isolates of S. apiospermum, most of which were isolated from clinical specimens. When the NCCLS M38-P microdilution method was used, itraconazole showed strong antifungal activities, while amphotericin B had little efficacy. A new triazole agent, voriconazole showed a strong effect against isolates of P. boydii and S. apiospermum MIC50 0.06 g ml , whereas micafungin, a newly developed echinocandin, had little effect MIC50 16 g ml There was no significant difference in the susceptibilities between P. boydii and S. apiospermum isolates against any antifungal agents. Our study suggests that voriconazole is a promising new drug in these infections, and that the same antifungal strategy can be employed in the infections by P. boydii and S. apiospermum. The in vitro susceptibility of Sporothrix schenckii to antifungal drugs has been determined with three different methods. Nineteen Peruvian clinical isolates of S. schenckii were tested against amphotericin B AB ; , flucytosine FC ; , fluconazole FZ ; , itraconazole IZ ; , voriconazole VZ ; , and ketoconazole KZ ; . Modified NCCLS M38-A, Sensititre YeastOne SYO ; , and ATB Fungus 2 ATBF2 ; methods were used to determine the MICs. ATCC isolates of Candida parapsilosis, Candida krusei, and Aspergillus flavus were used for quality control. Sporothrix inocula were prepared with the mycelial form growing on potato dextrose agar at 28 2C. MICs of AB, FC, FZ, and IZ were determined with all three methods, VZ with M38-A and SYO, and KZ with only SYO. The three methods showed high MICs of FZ and FC MIC90 of 0.5 g ml ; , being homogeneously lower than those of IZ and KZ. The M38-A method showed a variable MIC range of VZ 4.0 to 16 g the geometric mean GM ; was 9.3 g ml. The MIC range of AB was wide 0.06 to 16 g but the GM was 1.2 g ml, suggesting that the MIC is strain dependent. Agreement two log2 dilutions ; between commercial techniques and the modified M38-A method was very high with FZ, IZ, and FC. In AB and VZ, the agreement was lower, being related to the antifungal concentrations of each method. The highest activity against S. schenckii was found with IZ and KZ. Lack of activity was observed with FZ, VZ, and FC. When AB is indicated for sporotrichosis, the susceptibility of the strain must be analyzed. Sporotrichosis is a subacute or chronic infection that affects humans and other mammals and is produced by the dimorphic fungus Sporothrix schenckii.
