Design of commercial industrial irradiators may incorporate variations in the type of radiation source being used, the presentation of product to the radiation field, and methods for conveying product through the irradiator. In general, irradiator design is based on the product expected to be processed and on requirements for uniformity in dose delivery or volumetric throughput for the facility. It is estimated that IFIs develop in 10% to 25% of patients with acute leukaemia and those receiving BMT. The case fatality rate exceeds 50% and is approximately 90% in invasive aspergillosis IA ; , particularly in patients with persistent neutropenia. The role of the neutropenia for the development of IFI is well known and other risk factors include previous fungal infection, environmental exposure, long-term use of broad-spectrum antibiotics, steroids or chemotherapeutic agents and the use of high technology invasive monitoring and treatment modalities. Candida and Aspergillus species traditionally account for the majority of documented infections; however, recent epidemiological reports indicate a shift towards Aspergillus spp., non-albicans Candida and other emerging fungi such as Fusarium, Cryptococcus, Trichosporon, Rhizopus and Rhizomucor.
