Cancer care in Western Australia remains fragmented; some of the current problems have been described as poor communication between tertiary and community-based carers; poor resources and support for General Practitioners GPs the loss of involvement of the primary care practitioner in ongoing care and the high cost of tertiary care. Shared Care is an organisational model of care that involves primary care practitioners and specialists in the management of patients in a formal and explicit manner. This model was created in response to perceived inadequacies in current cancer management, as determined by GP surveys and interviews with stakeholder groups. The Haematology department at Sir Charles Gairdner Hospital ran a pilot of the model in 1999 2000; the pilot evaluation showed the following potential benefits: Continuity of care sharing care means that there is less confusion between care providers as to the overall plan of treatment. Convenience GPs will be able to manage some problems that may decrease the need for some visits to the tertiary centre. This may be more convenient for the patient. As the patient's GP may have known them and their family for some time, they may be in a more ideal position to provide ongoing support and also management of other health issues. Patients seeing their GPs regularly during cancer treatment may experience reduced severity of side effects due to early intervention. Patients may feel more involved in their care and may feel empowered by holding their own Patient Held Record.

Ulation of actin dynamics Van Alest and D'Souza-Schorey, 1997; Allen et al., 1998; Hall, 1998 ; . Because the actin cytoskeleton is important for cell morphology and motility, it is thought that Rho proteins affect cognition through regulating neuronal structures, including the dendritic spine, an actin-rich postsynaptic specialization in which the majority of excitatory synapses are formed Matus, 2000; Sorra and Harris, 2000; Bonhoeffer and Yuste, 2002; Ramakers, 2002; Lamprecht and LeDoux, 2004 ; . Accordingly, spine deficits are associated with many types of mental disorders, including MR Sorra and Harris, 2000; Ramakers, 2002 ; . However, in vivo studies of the MRX genes are currently limited. In this study, we generated and analyzed knock-out mice deficient in the expression of the MRX gene Pak3. The Pak3 gene encodes a serine threonine protein kinase that belongs to a family of p21-activated kinases PAK13 ; , the activity of which is directly stimulated by the Rho GTPase Van Alest and D'SouzaSchorey, 1997; Allen et al., 1998; Hall, 1998; Bokoch, 2003; Zhao and Manser, 2005 ; . To our surprise, we found that PAK3 knockout mice showed no deficits in either neuronal structures or the actin cytoskeleton. However, these mice were selectively impaired in late-phase hippocampal long-term potentiation LLTP ; , a distinct form of long-lasting synaptic plasticity involving new gene expression Bliss and Collingridge, 1993; Malenka and Nicoll, 1999; Kandel, 2001; Silva, 2003; Kelleher et al., 2004 ; . The knock-out mice showed a dramatic reduction in the active form of cAMP-responsive element-binding protein CREB ; , an extensively studied transcription factor required for activitydependent gene expression and L-LTP Kandel, 2001; Lonze and Ginty, 2002; Silva, 2003 ; . Furthermore, the PAK3 knock-out mice showed accelerated extinction of the hippocampusindependent implicit ; taste aversion associative learning task, whereas their hippocampus-dependent explicit ; spatial memory was not affected. These results indicate a novel role for Rho signaling in the regulation of CREB function and synaptic plas.

94.8%; calcium antagonist, 91.2%; diuretic, 87.5% ; as reported by Saito and Saruta.20 Furthermore, it was assumed that compliance would remain steady after 6 years of treatment. DBP was assumed to return to 100 mmHg if treatment was discontinued. Annual incidence of diabetes was estimated based on the 4-year incidence of diabetes in the ALLHAT21 by hypothesizing a Weibull distribution with shape parameter 0.5. The estimate of incidence of diabetes in the ACE inhibitor group was substituted for that among ARB users since the relative difference was similar in the VALUE study.22 The incidence of diabetes among patients taking combination therapies was assumed to be the mean of that associated with each individual drug. The incidence among patients who did not receive treatment was assumed to be the same as that among those patients using the calcium antagonist. ARB was assumed to decrease the transition probability to microalbuminuria and proteinuria by 24% based on the findings of MICRO-HOPE23 using ACE inhibitor, since ARB had a similar renoprotective effect as ACE inhibitor in patients with microalbuminuria, as shown in IRMA2.24. The present study is based on observations from seven pig-tailed macaques Mucaca nemestrina, 4-6 kg ; . These animals were also included in a study of corticospinal projections from the motor areas on the lateral surface of the hemisphere He et al., 1993 ; . A detailed description of the surgical and histological procedures we used to study these animals was presented in that publication and will only be briefly presented here. The use of the animals in this study followed the principles approved by the U.S. Public Health Service Policy on Humane Care and Use of Laboratory Animals PHS Policy ; , and the National Institutes of Health Guide for the Care and Use of Laboratory Animals NIH Guide ; . Surgical procedures We examined the topographic organization of corticospinal neurons on the medial wall of the hemisphere by injecting two fluorescent tracers, fast blue FB; 5% in distilled water ; and diamidino yellow DY; 2% in distilled water ; , into the gray matter of the spinal cord at different seg and pegvisomant. Cosmetics from the Baobab Tree -- Out of Africa Cognis, the same German-based multinational that patented Morocco's argan tree, has also staked out a claim to one of Africa's most widely-known plant symbols: the baobab tree. 102 ; The claims were first filed in France in 1997 and have issued in many countries, including the US. The patents cover use of baobab leaf extracts in "a cosmetic product for at least one of the skin, hair, eyelashes and nails of a human". 103 ; The baobab Adansonia digitata ; grows on savannas in much of Africa. It has a striking shape and a remarkable trunk, which stores water and can grow to 11 meters in circumference. It is difficult to overstate the cultural and symbolic significance of the baobab. Botswana's tourism ministry says, "Undoubtedly this magnificent colossus is the most frequently-discussed botanical phenomenon in Africa." 104 ; One of Senegal's best-known bands is Orchestra Baobab, and Cote d'Ivoire has dubbed an oilfield "Baobab". The Baobab even lent its name to characters in the Star Wars movie series. Cognis' "invention" is to use the baobab leaf mucilage as a soothing cream, termed an emollient in more technical language. 105 ; But baobab has a wide variety of traditional medicinal and other uses in Africa, including use of the leaves and use on the skin. Because the uses span most of the continent, a comprehensive review would take pages. One medicinal plant database has 51 citations describing traditional uses from Mauritania across West Africa, down to South Africa, and back north through East Africa to Ethiopia. 106 ; It seems most unlikely that Cognis was the first to discover the soothing effects of baobab mucilage when applied to the human body.

Social structures, social activities and social relations. The notion of learning being a social process of meaning making: making sense of the world, ourselves, our relationships with one another Scheeres, 1999 ; , leads to the idea that people can engage in learning to achieve a form of social action. They do so to exercise more influence on their world, using sociolinguistics to interpret the social world in which they live. Social Capital Interlinked with Lifelong Learning So, too, is social capital related to social structures, social activities and social relations surrounding learning. Putnam's 1993 ; social capital comes in the form of networks, norms and trust that facilitate cooperation for mutual benefit. It is self-reinforcing and is a stock that accumulates through use and expands as networks extend, strengthening communities. It is feasible that social capital becomes the modus operandi for sustaining lifelong learning, taking groups and individuals forward in their knowledge and practice. It is to the group that social capital belongs, not to any one individual. "Social capital is accumulated through interactions: interactions between individuals, between individuals and groups, and between groups". Kilpatrick, Bell & Falk, 1999: 131 ; . Knowing that learning occurs when individuals and groups interact Scheerees, 1999 ; , Falk 1997 ; connects social capital and learning by stating that "learning occurs through interactions between individuals, between individuals and groups and between groups of groups. The process of learning occurs in the context of the social capital of the community in which the individual, group or organisation learns" 131 ; . However, what is of utmost interest to me is the interconnection between social capital, lifelong learning and the quality of the sociolinguistic interactions learning conversations ; that must come together to generate quality learning experiences. Being in Conversation The capacity building role of social capital Topolsky, 1997 ; influences the lifelong learning process by enabling people to realise more of their individual potential, and by promoting ability and willingness of people to take action and make successful changes. The learning of individuals in groups is assisted by social capital because it becomes self-generating through the learning interactions it facilitates Kilpatrick et al, 1999 ; . However, there is also a need for `a way of being in conversation' that ensures the likelihood of creative, experiential learning. Social connections, networks and relationships nurtured through certain conversational spaces act as a resource to help people to access knowledge and advance their learning through cooperation with others, over time. These conversational spaces must be safe and broad enough to invite engagement while being robust enough to accommodate differences - keeping in mind they can be either a catalyst or an impediment to learning conversations Baker, Jensen, Kolb, 2002 ; . Social Learning Capital The learning that happens within certain conversational spaces, with their social connections and networks, is only as good as the ontological quality of the communication that may lead to meaningful learning and knowledge acquisition. It is the degree of enthusiasm, comprehension, reflection, action, genuineness of engagement, respect and even affection for others in the conversations, that impacts on the quality of the learning to bring about Social Learning Capital. The concept of Social Learning Capital is created through Interlinking social capital, informal lifelong learning and the idea of quality learning conversations. Social Learning Capital is the meshing of social capital and lifelong learning that reaches its potential through quality learning conversations and pennyroyal.
