Solution or voiding it. The reversibility of the surgical intervention is another advantage since the band can also be removed laparoscopically. The long-term efficiency is reduced when compared with malabsorptive techniques, especially in cases of extreme obesity BMI 50 kg m2 ; Brown et al demonstrated an 85% body weight loss, two years after LAGB in patients with BMI between 30 and 40 kg m2 Postoperative nutritional deficiencies are less frequent in patients who undergo LAGB if a balanced diet is followed. The long-term disadvantages of this technique include the need for careful monitoring of patient evolution, band adjustment, possible complications, band slippage, gastric wall erosion with intraluminal migration, complications related to implanting and securing the injection port. However, such situations rarely occur when an adequate surgical technique is employed 5 ; . Morbid obesity is associated with several specific comorbidities such as type 2 diabetes mellitus and insulin resistance, coronary heart disease, arterial hypertension, NASH, osteoarthritis, respiratory dysfunctions, etc. Morbid obesity significantly decreases the life expectancy of young patients. Constant weight loss favors the remission of diabetes mellitus in two thirds of diabetic morbidly obese patients. This therapeutic effect is obvious especially if surgery is performed 1-2 years after diabetes is diagnosed 3 ; . In our patient, the favorable evolution was proven by the halving of the insulin dose required and stable glycemia levels, 4 months postoperatively. Nonalcoholic steatohepatitis in morbidly obese patients is signaled by increased transaminase and gamma-glutamyltransferase levels, insulin resistance, hepatomegaly, hepatic steatosis as revealed by ultrasound examination in patients who were not given hepatotoxic drugs or alcohol and had negative viral and autoimmune markers 6 ; . It frequently occurs in combination with metabolic syndrome components. Most studies indicate that simple steatosis does not usually turn into NASH or cirrhosis. On the other hand, 30-40% of NASH cases turn into fibrosis over a 4year period. The risk factors for such an evolution include obesity, type 2 diabetes mellitus, increased tranaminase levels AST ALT 1 ; and age over 45 years 6, 7 ; . The therapy of NASH is usually targeted at the associated disorders: obesity, diabetes, dyslipidemia. A slow reduction in body weight 1.5 kg week ; proved efficient in both reducing transaminase levels, and improving histological liver changes 7 ; . The liver ultrasonography carried out 2 and 4 months postoperatively did not detect changes as compared with the preoperative investigation. The metabolic syndrome is frequently associated with chronic inflammatory syndrome and coronary heart disease.
Abstract Withdrawn POS-03.76 Bone mineral density in men undergoing prostate biopsy Leocadio DE, Mandalapu S, Stein BS Department of Urology, Rhode Island Hospital Brown Medical School, Providence, RI, USA Introduction: This study was undertaken in order to determine whether bone mineral density BMD ; is lower in men at the time of diagnosis with prostate cancer CAP ; vs. age matched men without a biopsy revealing cancer, while controlling for testosterone level. Methods: Dual energy x-ray absorptiometry DEXA ; scans of the femoral neck and serum osteocalcin levels were used to quantify bone turnover in 58 patients at the time of initial prostate biopsy. Total serum testosterone levels were also obtained. BMD and serum assays of patients diagnosed with cancer n 31 ; were compared to those whose diagnosis was benign n 27 ; . Results: No significant difference was found between the Cancer and No-Cancer groups in terms of mean BMD T-score -1.04 1.08 and -1.07 1.08, respectively; p 0.91 however, the mean of both groups was in the osteopenic range. No statistically significant differences were found between the Cancer and No-Cancer groups when comparing mean testosterone levels 396 139 and 407 166 ng dl, respectively; p 0.99 ; alone, nor when testosterone was compared across BMD diagnoses for the Cancer p 0.88 ; and No Cancer p 0.67 ; groups. Mean ages for the Cancer and No-Cancer groups were 64.0 1.6 and 62.4 1.4 years, respectively p 0.462 ; . Conclusions: When controlling for age and testosterone levels, there was no difference in BMD in patients with or without CAP. Both groups, however, had average BMD levels in the osteopenic range T-score -1.04 ; . Two nested pairs of primers were used to successfully amplify M. leprae DNA from the lepromin skin test reagent, samples from treated leprosy patients with a BI of zero, and from three of six archaeological specimens dating from the 10th11th centuries. Nested PCR increases the sensitivity of PCR as it enables a total of 50 amplication cycles to be used, but avoids over-amplication artefacts. It was only with nested PCR based on the RLEP primers that all three clinical samples were found to give positive results. It is not known whether or not residual M. leprae DNA is of any clinical signicance, so it may be of interest to use these primers, which can detect lower amounts of M. leprae DNA than current methods permit, in longitudinal studies of treated leprosy patients. All three positive archaeological samples were obtained from the cavum nasale. It is known that M. leprae localises to Schwann cells and nasal epithelial cells [19], and that this can result in an overwhelming rhinitis associated with very high numbers of the organism. The three cavum nasale specimens demonstrated the typical palaeopathology associated with leprosy, so it was likely that M. leprae DNA was in these samples at the time of death of the host. The detection of M. leprae in specimens .1000 years old was described previously in a preliminary report [15] and these additional observations, plus DNA sequencing data, conrm the persistence of M. leprae DNA. Leprosy is primarily a disease of peripheral nerves and skin, but affects bones as well. However, the absence of detectable M. leprae DNA in the metatarsal specimens A2, A5 ; and the bula sample A6 ; need not imply that the remains were from individuals who were not suffering from the disease. Although many skeletons have been found with changes associated with lepromatous leprosy, the peripheral bones are likely to contain relatively little M. leprae DNA compared with the nasal region. Furthermore, most observers consider the changes in the limbs to be due to secondary infection, which is the result of loss of sensation due to nerve damage by the organism [20], so some affected limbs may contain no M. leprae DNA at all. All the new primers were of greater sensitivity than the commonly used primers described by Hartskeerl et al. [2] and enabled the M. leprae-positive archaeological specimens to be detected by single-stage in addition to nested PCR. One reason for this difference is the smaller target size, as this is more likely to result in successful PCR assay of damaged and fragmented DNA. The most sensitive primers were based on the repetitive sequence RLEP ; , which is repeated 28 times in the M. leprae chromosome. The primers based on and nettle.

Portion as much as possible and improving the form layout on the MHAUS Web site. Update on Molecular Genetics Dr. Sheila Muldoon announced guidelines have been established for pursuing genetic testing that will someday hopefully spare families from the inconvenience and expense of the contracture test. She believes this new direction will give the biopsy centers a new focus and give more patient satisfaction. At this time, the genetic test is not sensitive enough and will not replace the CHCT test. Dr. Muldoon explained we need to continue the research to increase the accuracy of the test for it to be value. Dr. Rosenberg then discussed a possible molecular genetic testing site and indicated the cost for initial screening should be about 00. He recommended the Web site "genetests " run through NIH funding ; which contains clinical and molecular genetics information on a number of disorders. Public Relations Update Al Rothstein provided a packet of media stories on MH. Dr. Rosenberg commended Mr. Rothstein's strong efforts and asked those in attendance to pass along items of interest to Al. QA Committee Update The MH Hotline Consultants provide expertise; trained nurses and physicians are available around-theclock to answer urgent calls and general questions about management of suspected MH cases. In order to assure consistent quality advice, new Consultants are matched with an experienced Consultant and mentored during the first year of service. Each quarter, the Quality Assurance committee reviews and reports on 10 random cases. Committee Chair Charles Watson, MD, reported, "Although individual management styles and opinions differ, overall advice is consistently good. To conform to HIPAA guidelines, patient information is noted but not shared without patient permission." s and neulasta.
I pleased to learn that the 22nd General Conference of the World Fellowship of Buddhist is being held in the beautiful city of selangor, Malaysia. The theme of the General Conference "Compassion and Wisdom - The Way Forward" is highly relevant in today's world faced with challenges of poverty, environmental degradation and threats of war and terrorism. While Mankind has made tremendous scientific and technological advancements in all spheres, our world continues to face challenges of confusion, distress and conflict. Greed, anger and violence fuel an endless cycle causing more of the same poisons in which we are mired. While some are caught in the vicious cycle of waste and over consumption, others are fighting for crumbs, unable to meet the bare minimum needs for subsistence themselves and their loved ones. Mankind's violence is not confined to each other or to other sentient beings but threatens the very survival of our planet. Lord Buddha's teachings are, therefore, a beacon of hope for curing the past, present and future ills of mankind. It is in this context that such a General Conference of the World Fellowship of Buddhist assumes great significance and relevance. For Bhutan, where spiritual well-being is an important element of its national goal - "Gross National Happiness", we would like to lend every support to the noble activities of fellow Buddhists all over the world. Bhutan became a member of the World Fellowship of Buddhists during the 14th General Conference held in Colombo, Sri Lanka in August 1984. The convergence of Bhutan's national polices - particularly, those related to culture, religion, environment, respect for human rights and dignity, and love and compassion for all sentient beings - with that of the noble goals of the World Fellowship of Buddhists has assured us the correctness of the direction we are following and the impetus to follow the set path with greater confidence and zeal. The people of Bhutan deeply appreciates the noble endeavor of the World Fellowship to bring peace and harmony in the world through the dissemination of Lord Buddha's message of Peace and harmonious co-existence of all beings. May all your prayers and actions spread the teachings of the Lord Buddha and may they result in the blossoming of compassion and loving kindness, which alone will bring the healing balm of peace, love and happiness all over troubled world.
Figure 8: Proposed model for the mechanism of disruption of Ca2 + homeostasis by mefloquine and its similarities with ionomycin. A ; Based on the protective effect of neomycin and DNQX we initially suspected that mefloquine activated non-NMDA receptors and the G protein-PLC-IP3 pathway. This possibility was ruled out based on receptor-binding studies and the fact that calcium homeostasis was similarly disrupted in non-neuronal cells macrophages ; that do not possess non-NMDA receptors. B ; Mefloquine and ionomycin exhibit similar downstream effects on the disruption of cellular calcium homeostasis by releasing Ca2 + from ER stores and, as a result, triggering a CCE response. The relationship between the store discharge and the known and putative ionophoric properties of ionomycin and neupogen.
Secondly, differing from Piaget's theory, it is presumed that the basic structures of logico-mathematical thought are not determined epigenetically, but are developed by the individual growing up in confrontation with culture-specific challenges and constraints under which the systems of action have to be internalized. The challenges are embodied by material means of goal-oriented or symbolic actions that are shared external representations of the logico-mathematical structures. Thus, according to this assumption, the cognitive structures, according to which logico-mathematical competence is defined, are in ontogenesis not construed independently from processes of socialization, but have as their constitutive condition the co-construction of cognitive structures by means of interaction and communication. Such co-constructions on the micro level of social interaction make it possible that cognitive structures are transferred from one individual to the next and so, on the macro level of social development, are transmitted as intersubjectively shared schemata of interpretation. This process gains historical continuity through collective external representations which, as will be shown, embody both cognitive structures and levels of reflection, and thus levels of abstraction. The first of these two assumptions formulates a developmental-psychological, the second a knowledge-sociological precondition for a theory of the historical development of logico-mathematical thought. By combining both assumptions, a twofold result is achieved. On the one hand, psychological theories and conclusions receive a culture-historical interpretation; on the other, historical stages of the development of thought can be characterized psychologically. As will be shown in the following, particularly in the case of the development of arithmetical thought which is the subject of the present study, the historical stages of this development can be interpreted as subsequent meta-cognitive levels that are connected with each other by reflective abstractions.
AUTHORITY OF THE COMMITTEE. Prior to the Amendment Date, this Plan constituted a formula plan for purposes of Rule 16b-3, thus the Committee's authority was restricted as required to maintain that classification. Upon and after the Amendment Date, to the extent necessary to apply the provisions of subparagraph a ; II ; of Section 4, this Plan is deemed not to be a formula plan. Because only nonemployee directors are eligible to receive a grant of an Option under this Plan, all Options granted under this Plan shall constitute options which are not incentive stock options as defined under Section 422 b ; of the Internal Revenue Code of 1986, as amended. Options which shall be granted hereunder are hereinafter referred to as "Nonqualified Stock Options". ; The Committee may interpret the Plan, prescribe, amend and rescind any rules and regulations necessary or appropriate for the administration of the Plan and make other determinations and take other action as it deems necessary or advisable. Without limiting the generality of the foregoing sentence the Committee may, in its discretion, treat all or any portion of any period during which an Optionee is on military or an approved leave of absence from the Corporation as a period of employment of the Optionee by the Corporation, as the case may be, for purpose of accrual of his rights under his Option. An interpretation, determination or other action made or taken by the Committee shall be final, binding and conclusive. d ; INDEMNIFICATION OF COMMITTEE. In addition to other rights that they may have as Directors or as members of the Committee, the members of the Committee shall be indemnified by the Corporation against the reasonable expenses, including attorney's fees actually and necessarily incurred in connection with the defense of any action, suit or proceeding, or in connection with any appeal therein, to which they or any of them may be a party by reason of any action taken or failure to act under or in connection with the Plan or any Option granted thereunder, and against all amounts paid by them in settlement thereof or paid by them in satisfaction of a judgment in any such action, suit or proceeding, except in relation to matters as to which it shall be adjudged in the action, suit or proceeding that the Committee member's action or failure to act constituted self-dealing, willful misconduct or recklessness; provided that within sixty 60 ; days after institution of any action, suit or proceeding a Committee member shall in writing offer the Corporation the opportunity, at its own expense, to handle and defend same. 4. Ethylammonium raises pressure in the former and lowers it sharply in the latter. The depressor action of azide is essentially unchanged regardless of what chemical was infused to produce the acute hypertension. This again suggests that as long as the muscle receptors are not saturated as a result of the continuous infusion of the drug, they are able to respond normally, no matter how the blood pressure was raised. The responses to barium chloride are also consistent with this view. The response to veratrum Veriloid ; was little changed during acute hypertension except in hypertension due to angiotonin and increased intracranial pressure where it, was somewhat reduced. That veratrum could still initiate the von Bezold reflex and decrease sympathetic vasoconstriction during periods of acute hypertension is surprising and emphasizes the remarkable ability of the body to hold protective mechanisms in reserve, even under adverse hemodynamic conditions. Although the results of this investigation may find application in the analysis of reactivity of hypertensive patients, we shall not at present discuss the problem, but certain special cases may be mentioned in which the physiologic mechanisms receive some clarification. Mayock and Rose2" pointed out that before the removal of a functioning pheochromocytoma, patients are insensitive to adrenaline. We have observed insensitivity to the infused drug when acute hypertension has been established which is not extended to other, chemically unrelated, pressor or depressor substances. The profound depression of blood pressure after discontinuing infusions of adrenaline and noradrenaline has been noted by many observers and called "adrenaline shock." Our animal experiments agree with those of Green and associates2' on patients in the rough correlation between the height of the initial pressure and the depth of the postinfusion depression. As estimated by this, vasodilator capacity in hypertensive animals is certainly not impaired. From a small experience with dogs in the malignant phase of hypertension, sensitivity increases early to be followed by progressively waning response as death approaches. Myotropic substances such as angiotonin and and nicardipine.
