The Cox-2 class of drugs, which as you know all too well, includes Vioxx, Celebrex, and Bextra, continues to receive heavy scrutiny in the medical community.Although Vioxx and Bextra are now off the market, with Celebrex having a black box warning in its label indicating the increased risk of heart attacks and strokes, there is even more bad news for the drug companies that put these drugs on the market. Frankly, I have been shocked that the manufacturers of the Cox-2 drugs have continued to deny liability for the thousands of lawsuits that have resulted in serious injuries or death associated with the ingestion of these drugs. To date, there have been several Vioxx trials that have led to jury awards for victims ranging from million to 3 million. The latest decision was in the New Jersey case referred to above. Interestingly, there have been no Celebrex or Bextra lawsuits to reach trial yet. Along with the jury verdicts, there has been an increasing body of clinical trials.
Assessed using a semiquantitative scale in 4 brain regions ie, hippocampus, midfrontal cortex, inferior parietal cortex, and superior temporal cortex ; and an average score was computed for each case. Results: We found that severe CAA was associated with an increased frequency of VLs 33% of the cases of severe CAA had VLs vs 19% of the cases of mild or absent CAA: P .02 ; . While the APOE4 4 genotype was associated with an increased severity of CAA, there was no significant relationship between APOE genotype and frequency of VLs. Logistic regression models showed that severe CAA, advanced age, atherosclerosis, and Hachinski Ischemia Scale score of 7 or more were all significantly associated with VLs, but the number of APOE4 alleles, history of hypertension, coronary artery disease, sex, and serum cholesterol levels had nonsignificant effects. Within strata of APOE genotype, the presence of severe CAA was associated with increased frequency of VLs eg, within APOE4 4 homozygotes. VLs were present within 47% of the cases of severe CAA vs 9.5% of the cases of mild or absent CAA: P .01 ; . Conclusions: Severe CAA confers a greater risk of VLs in AD, even within strata of APOE genotype. Therefore, the association between severe CAA and VLs in AD is not a spurious one owing to APOE4. Overall, our cases of AD with APOE4 do not seem to be a more "vasculopathic" subtype of AD. The mechanisms by which CAA produces VLs of various types need to be further elucidated, as these are probably important in producing the common entity of "mixed" AD vascular dementia.

Updated data from 52 randomized clinical trials30 showed no evidence that any group of patients specified by age benefited more or less from adjuvant chemotherapy. A recent retrospective analysis evaluated the influence of age on survival, chemotherapy delivery and toxicity in the above-mentioned positive NCI-C BR10 trial.41 Pretreatment characteristics and survival benefit from treatment were compared for patients 65 years of age and less and those over age 65. Data from 327 young and 155 elderly patients were included in the analysis. Baseline prognostic factors by age were similar with the exception of histology adenocarcinoma 58% young, 43% elderly; squamous cell 32% young, 49% elderly; P .001 ; and performance status PS 0 53% young, 41% elderly; P .01 ; . Overall survival by age showed a trend favoring the young in univariate HR for death 0.77; P .084 ; and multivariate analysis HR for death 0.75; P .059 ; . Patients aged 75 years or more had significantly shorter survival than those aged 66 to 74 for death 1.95; P .02 ; . However, overall survival for patients greater than age 65 was significantly better with chemotherapy than with observation HR for death 0.61; P .04 ; . The elderly received significantly fewer doses of chemotherapy both cisplatin and vinorelbine ; . Thus, this retrospective study suggests that despite receiving less chemotherapy than younger patients, those aged 65 or greater achieved improved overall survival with adjuvant chemotherapy with acceptable toxicity and nembutal.
There have been studies in Hodgkin's disease which have looked at different ways of combining chemotherapy and radiation. Currently the emphasis is on investigating novel drug combinations in the treatment of non-Hodgkin's lymphoma. Mabthera is a monoclonal antibody which targets B lymphocytes and has been shown to have synergistic effect with conventional chemotherapy. There have been a series of trials looking at combinations of Mabthera with various types of chemotherapy. There has also been one important clinical trial looking at the role of Mabthera as maintenance therapy for patients who have already achieved remission after conventional treatment. A novel drug combination of Gemcitabine, Navelbine and Ifosfamide is currently being tested in a clinical trial which is directed from The Alfred Hospital. Gastric Cancer Recent clinical trials looking to see whether the prognosis of patients who have undergone potentially curative surgery could be improved by the addition of chemotherapy. Some of these trials have been completed but other trials are due to start shortly. A new drug combination with docetaxel and gemcitabine will commence shortly to test a group of patients with gastric cancer who have a more advanced form of the disease.
The 24 patients included in the study were selected from 623 successfully karyotyped, out of the 809 patients included in LALA trials over a 6-year period 751 patients in the LALA 94 and 58 in the LALA SA ; . They presented either a hypodiploidy 30-39 chromosomes 11 patients ; or a near-triploidy 13 patients and neomycin.
The main objective of this study is to describe a group of patients with nsclc treated at first-line with navelbine as a single-agent, to identify those having a survival time greater than or equal to 18 months, and then to perform a multivariate analysis of the prognostic factors affecting long-term survival.

In memory and other cognitive functions.1 Several pharmacological approaches have been used in an attempt to correct these deficits, including increasing the synthesis of acetylcholine, activation of muscarinic or nicotinic acetylcholine receptors, and inhibition of acetylcholinesterase, the enzyme responsible for the hydrolysis of acetylcholine.2 Of these strategies, inhibition of acetylcholinesterase is currently the most successful treatment for Alzheimer's disease.3 Well designed clinical trials have consistently shown improved cognition and global assessment scores in patients taking acetylcholinesterase inhibitors.3 4 The effects of cholinesterase inhibitors on patients' activities of daily living are unclear.5 6 There is also some evidence that patients who have the apolipoprotein E4 genotype may have a reduced response to cholinesterase inhibitors.7 8 Galantamine is a new drug that reversibly and competitively inhibits acetylcholinesterase9 10 and enhances the response of nicotinic receptors to acetylcholine.11 This enhancement of nicotinic neurotransmission may be clinically relevant because activation of presynaptic nicotinic receptors increases the release of acetylcholine and other neurotransmitters, such as glutamate, that are deficient in patients with Alzheimer's disease.12 13 We evaluated the efficacy and safety of two maintenance doses of galantamine over six months compared with placebo in patients with mild to moderate Alzheimer's disease. We also investigated whether the apolipoprotein E4 genotype influences the response to galantamine.

Authors' reply Editor--To suggest, as Joseph and White do, that regional and the central advisory committees on distinction awards are beyond reproach is ignoring the huge amount of evidence that we have collected on the racism of the medical profession.1 No one is exempt, and the senior members of the profession, because they are in a position of power, must bear the brunt of the responsibility. It is scandalous that an organisation that disburses 185 million of public funds does so in a climate of secrecy with little regard to issues of fairness and equality which affect all doctors and not just the 20% of ethnic minority doctors that are highlighted in our report. We need better information, which as Bedi and Williams point out is essential to our understanding of what is going on. But more information will not resolve the issue until the profession accepts that racism is a problem and develops mechanisms to deal with it. As we have pointed out previously, racism affects students when they apply for medical schools, 2 in career progress, 3 in complaints, 4 and now in remuneration. We believe that a major reform of the merit awards system is required. General practitioners and other senior doctors such as staff graders, who provide a huge and often unrecognised service to the NHS, should be considered for merit payments. Explicit criteria for merit need to be developed. Patient care as well as research needs to be taken into account, as should total earnings. For example, why should consultants who have a lucrative private practice be rewarded when a lot of senior.
