This month INTAL published its first Andean Report with a view to offering a wide overview of this integration process and thus continue along the line of the sub-regional studies on integration which was successfully launched through the MERCOSUR Report. The publication of this document is made at a time when the Andean Community member countries are moving forward towards the consolidation of a free trade area and the implementation of a four-level common external tariff CET ; by the end of 2003. The challenges and opportunities of this crucial moment of Andean integration require efforts that will contribute to understanding the complexity of this multidimensional phenomenon and the sustainability of these undertakings that will drive the future of these countries' international insertion. The Andean Report is divided into six chapters. The first describes the main characteristics regarding the transition of the Andean Group towards the Andean Community CAN ; , based on the institutional changes that took place during the process, as well as on its driving factors. The second chapter aims at globally assessing this integration process, by identifying its strengths and weaknesses, and takes into consideration some key variables, which are then discussed more thoroughly in other parts of the Report. Occasional Paper SITI-10 published by INTAL this month, within the framework of the IDB Special Initiative on Trade and Development, addresses the entry of ten new countries into the European Union EU ; and its consequences on relations with MERCOSUR. The paper written by Renato G. Flres Jr. reviews available evidence on the enlargement of the EU, by using impact evaluations and tools to analyze partial and general balances. The author considers that the May 2004 enlargement is not a matter of concern in general- for MERCOSUR decision-makers and economic agents. Nonetheless, a more detailed outlook does give rise to a few concerns which call for preventive political measures. With regard to the prospective benefits that an enlarged EU can bring about for MERCOSUR the study finds that, although many South American products would gain, certain market segments would be lost such as footwear, leather, chemical products and automobiles. In other sectors, like steel or machines, it is expected that there will be adjustments though the final outcome will probably be positive for MERCOSUR countries. If the added well-being gains are taken into account, the enlargement is considered positive, under a scenario of either complete or incomplete liberalization of MERCOSUR-EU 25 trade, even if there are losses in the abovementioned sectors. According to Flres Jr., the added value can be construed by stating that the algebraic sum of losses and gains will in time ; favor MERCOSUR. The study considers the possibility for countries that joined the EU recently to penetrate more deeply compared to the EU 15- into some MERCOSUR market segments. Such segments would focus on a few traditional manufactures and, maybe, on some with a greater technological content. Indeed, the increase in existing competitiveness in the EU 15 will be gradually sensed in automobiles, chemical and electronic products, with certain influence on a few agricultural commodities. Regarding distortions that can be expected in the Common Agricultural Policy CAP ; within the enlarged EU, it is estimated that it will take time before it becomes operational in the countries which joined recently. This will give MERCOSUR countries extra time to get ready. Nonetheless, even in the case of a full implementation of the CAP and presupposing that the EU does not implement a new "creative scheme" for this policy- this mechanism is not expected to eliminate growing gains in the MERCOSUR agribusiness sector. Finally, the Document addresses the topic of enlargement and its effects on a prospective EU-MERCOSUR free trade area, and holds that such area does not appear to be too distortive with regard to multilateral trade liberalization. In this regard, it adds that the above effects would be naturally diminished by such enlargement. However, the document also addresses a topic that sooner or later must be faced by MERCOSUR, beyond the free trade agreement with the EU: the bloc can become essentially consolidated as an exporter of agricultural products and commodities. By August 2007, the New York Times reported that there were at least 45, 000 lawsuits against Merck.85 The Michigan Vioxx victims, however, were unlikely to have a judge or jury decide their case because of the drug immunity law.86 In November 2007, Merck offered to pay .85 billion to settle the tens of thousands of pending cases from people who were harmed by taking Vioxx.87 Michigan residents initially thought that they would be left out of the settlement because of the immunity law, 88 fortuitously for these residents, the New Jersey judge overseeing the roughly 1, 000 Michigan Vioxx claims had not yet thrown out the cases before Merck offered the settlement.89 It seems clear that had Merck not offered a settlement, it would have been unlikely that the Michigan residents would have ever seen their cases in court and isdn.
Nitric oxide treatment has been very widely used in adults with acute lung injury ALI ; or acute respiratory distress syndrome ARDS ; for fifteen years, and it often improves oxygenation at least marginally for at least a few days. However a whole series of relatively small randomised trials have failed to provide evidence that use improves survival, and meta-analysis of 12 trials involving 1237 patients published in 2007 showed that while the PaO2 FiO2 ratio improved for a time, it delivered no reduction in pre-discharge hospital mortality, or time on a ventilator, and there was some evidence that use actually increased the risk of renal dysfunction Achikari et al., 2007 ; even when the dose used is kept below 20 ppm. On the basis of this evidence the routine use of nitric oxide in adults has now started to decline. Nitric oxide is however now accepted as being the most effective pulmonary vasodilator in babies of more than 34 weeks gestation. While nitric oxide use does not seem to improve survival, it does reduce the need for ExtraCorporeal Membrane Oxygenation ECMO ; . Neither of these strategies should be delayed until the baby is in extremis, and prompt referral to a centre capable of providing ECMO should be considered if the oxygenation index OI ; approaches 300, [or 40 if arterial oxygen pressure pO2 ; is being measured in mmHg rather than kPa], and does not fall at least 20% after 4 hours of treatment with nitric oxide.
