The major action of bile acid sequestrants is to lower LDL cholesterol.12, 13, 829-832 Therapy with cholestyramine reduced the risk of CHD in the Lipid Research Clinics Coronary Primary Prevention Trial.12, 13 Beneficial outcomes also occurred in other clinical trials in which sequestrants were combined with other lipid-modifying drugs.157, 158 Sequestrants add to the LDL-lowering effects of other drugs, notably statins.833-835 They remain unabsorbed in their passage through the gastrointestinal tract and lack systemic toxicity. Their disadvantages are two-fold. Because of their bulk, they lack convenience of administration; they also cause various gastrointestinal symptoms, notably constipation. The sequestrants bind bile acids in the intestine through anion exchange; this binding reduces the enterohepatic recirculation of bile acids, which releases feedback regulation on conversion of cholesterol to bile acids in the liver. The resulting decrease in hepatocyte cholesterol content enhances LDL-receptor expression, which in turn lowers serum LDL-cholesterol concentrations.836 In some persons, sequestrants increase hepatic VLDL production, 837 thereby raising serum triglyceride levels.838 Cholestyramine and colestipol are both administered as powders that must be mixed with water or juice. They usually are given once or twice daily with meals. Colestipol also comes in 1g tablets. The LDL-cholesterol-lowering effect of 4g of cholestyramine equals that of 5g of colestipol. Eight to 10 g day cholestyramine or 1020 g day colestipol reduce LDL-cholesterol concentrations by 1020 percent. Smaller doses of sequestrants 810 g day ; generally are well-tolerated; higher doses 1620 g day ; are less well-tolerated. Colesevelam, a recently marketed drug, is a much more potent bile acid sequestrant. It has been primarily evaluated at doses of 2.63.8g day, and reductions in LDL cholesterol of 1218 percent are reported.831 Colesevelam is more easily administered and better tolerated than other sequestrants. Sequestrants add to LDL lowering when combined with other cholesterol-lowering drugs. Whereas doubling the dose of a statin produces only a 6 percent further reduction in LDL cholesterol, adding a.

These BASs also effectively reduce LDL cholesterol levels, 25 but are often poorly tolerated and have significant drug interactions.11 Colesevelam HCl is a specifically engineered BAS that has also shown efficacy in reducing LDL cholesterol levels and is generally well tolerated, with fewer drug interactions than seen with conventional BASs.11 These studies were conducted to determine the effects of colesevelam HCl plus stable statin therapy with regard to the mean percent change in the levels of LDL cholesterol, other lipid parameters, and hs-CRP. All 3 presented studies were similar in design and end points assessed. A limitation to these studies was that an unexpected, unusually large, favorable statin-plus-placebo effect on lipid variables was observed in 2 of the studies, which helped account for the lack of statistical significance observed for some of the lipid parameters in the colesevelam HCl plus statins group when compared with the placebo plus statin group. These finding were at odds with previous trials of colesevelam HCl and statin combination therapy in which the combination significantly reduced LDL cholesterol levels more than statins alone, 1114 and the unanticipated reduced effect on lowering LDL cholesterol levels compared with placebo likely contributed to the lack of statistical significance found in the effects of colesevelam HCl on apo-B levels in the pooled analysis. A potential confounder was that although patients were required to be on stable statin dose to meet the entry criteria of the study, the statin dose and stabilization before study entry were not confirmed beyond the patient reports. It is thus possible that the unexpected, large reduction in LDL cholesterol levels in the statin-plus-placebo group could have been due to improved statin compliance once the patients entered the trial, even if such improved compliance involved only a few study participants, given the relatively small number of subjects in the statin-plus-placebo groups. Additionally, statin compliance was not formally assessed throughout the trials, and LDL cholesterol levels are known to vary for reasons other than the type and dose of statins prescribed, including issues regarding patient compliance and adherence to therapy.15 In contrast to some of the lipid findings in the individual trials of colesevelam HCl plus a statin, data from the pooled analysis revealed that the addition of colesevelam HCl to and comfrey. Our results indicate that neural activity in the magnocellular red nucleus correlates closely with the activation of one or more limb muscles, particularly with distal muscles. RNm discharge also correlates, though less consistently, with movement velocity. These conclusions are based on simultaneous recordings of EMG, kinematic and singleunit data, collected while monkeys performed a steptracking task. We found that RNm-EMG cross-correlation functions had larger peak values than either RNm-velocity or RNm-position functions. Typically, velocity signals only correlated well after they had been rectified, an operation that made them dynamically more like the EMG signals. Detailed analysis of cases with high correlations indicated that unit discharge tended to predict both EMG and velocity signals. However, when EMG and velocity were dissociated by the behaviour of the animal, RNm discharge followed a different time course to the velocity signal; this altered time course closely paralleled that of the EMG signal. Finally, when opposite directions of movement were tested, very few units responded reciprocally. The frequent bidirectional or unidirectional patterns of unit discharge were paralleled by similar patterns of muscle activation and cosopt.

Amounts of caffeine were ingested in either unsweetened coffee or 24 ounces of sugar-free cola in a controlled clinical trial, there was no effect of vehicle on the magnitude of peak caffeine concentration or on selfrated subjective effect scales.43 C a f feine absorption has been reported to be more rapid from capsules compared to cola or chocolate.44 Caffeine is widely distributed in body tissues. In the rat, expos u re to low dose of caffeine during gestation and postnatal life had on ly minor effects on the deve l o pment of adenosine A and A2a re c e ptors and GABA-A receptors in the rat bra i n .45 Other re p o rts have suggested that ch ronic ca f feine intake altered total brain weight, and DNA and RNA content in various brain re g i ons in developing rats compared with con t rol animals. 46 The elimination of caffeine from the body occurs mostly by metabolism.47-51 The urinary recovery of unchanged caffeine is only 14% following a single oral dose. The metabolic degradation pathways and pharmacokinetics of caffeine have been well studied. The metabolism of caffeine 1, 3, 7-trimethylxanthine; Figure 1 ; is complex and at least 25 metabolites have been identified in humans. The main route of elimination is the formation of 1, 7 dimethylxanthine paraxanthine ; by demethylation which is catalyzed by cy tochrome P-450 CYP1A2 ; . This enzyme also mediates other demethylation pathways to produce 3, 7 dimethylxanthine and 1, 3 dimethylxanthine. Overall, CYP1A2 accounts for approximately 95% of caffeine metabolism. A relatively small percentage of a dose 36% ; is converted into theophylline and theobromine. The metabolism of caffeine appears to be dose-dependent with repeated dosing.52 The nonlinear accumulation of methylxanthines in the body may partially explain increasing anxiety and nervousness when excessive amounts of caffeine are consumed. The elimination half-life of ca f feine is highly variable with a mean value of 5 hours but with a range of 212 hours for most individuals. In neon a t e the elimination half-life may range up to 100 hours. Since CYP1A2 can be both induced and inhibited, various phys i o l gical and environmental factors can influence the disposition of ca ffeine. For example, prior to the onset of menstru a t i caffeine elimination may be slowed in the late luteal ph a s e, potentially leading to exaggerated ph a rmacological effects during this time.52 Caffeine's and creatine.
