SURG. 2006 22 3 ; - summ in ENGL PURPOSE: A small percentage of cases with essential blepharospasm or hemifacial spasm will become resistant to botulinum toxin A Botox ; . We present our experience treating these patients with botulinum toxin B Myobloc ; . METHODS: We reviewed all charts of patients in one physician's practice who received botulinum toxin B after becoming refractory to botulinum toxin A. For each treatment session, patients were evaluated for side effects, relief of spasms, and duration of treatment effect. RESULTS: Data were collected on 16 patients and a total of 93 treatment visits. Average total dosage was 3, 633 U per treatment session. Mean duration of beneficial effect was 7.3 weeks and was most commonly rated as fair to excellent. Side effects for botulinum toxin B occurred at a higher rate than is typical for botulinum toxin A. Most common side effects were pain on injection 100% ; , ptosis 32.3% ; , facial or mouth droop 22.6% ; , dry mouth 17.2% ; , dry eye 9.7% ; , exposure keratitis 7.5% ; , and blurred vision 5.4% ; . The rate of occurrence of side effects appears to be dose-dependent. CONCLUSIONS: Botulinum toxin B is a useful treatment for cases that have become refractory to botulinum toxin A. The duration of beneficial effect is shorter than for type A. The side effect profile is similar to that for toxin type A except for pain on injection and the occurrence of dry mouth. Side effects occurred at a higher frequency than with botulinum toxin A, and the pattern suggests that botulinum toxin B spreads more diffusely. Side effects appear to be dosagerelated. 2006 The American Society of Opthalmic Plastic and Reconstructive Surgery, Inc. 719. The clinical potential of a point-of-care assay to assess interindividual variability in platelet aggregation among patients taking clopidogrel - McLean D.S. and Cannon C.P. [Dr. D.S. McLean, Department of Internal Medicine, Brigham and Women's Hospital, 75 Francis St, Boston, MA 02115, United States] - CRIT. PATHWAYS CARDIOL. 2006 5 2 ; - summ in ENGL Clopidogrel is an important component of medical therapy for patients with acute coronary syndromes and those receiving coronary artery stents. Despite use of clopidogrel, a significant number of patients experience recurrent adverse ischemic events. Interindividual variability of platelet aggregation in response to clopidogrel may be an explanation for some of these recurrent events, and small trials have linked "clopidogrel resistance" as measured by platelet function tests to adverse events. Additionally, the degree of clopidogrel-induced platelet inhibition appears to be a factor determining bleeding risk at coronary artery bypass grafting. A point-of-care device that could accurately and rapidly measure the degree of platelet inhibition among patients taking clopidogrel could be clinically valuable. Such a device would have the potential to Section 38 vol 42.2. Specialty physicians. General Observations Infrared Spectrum-The infrared spectrum of the acidic glycolipid fraction from the upper phase obtained by Folch partitioning showedstrong OH absorption at 3400 cm"; typical absorptions of amide I and amide I1 around 1640 and 1550 cm"; neither absorption of a sulfate S O band around 1245 cm" and a weak band at 815 cm" nor that of an ester such as 0-acetylated sugar. A sample of the acidic glycolipid fraction was also subjected to the quantitativesodium rhodizonate test 24 ; , with negative results affirming the absence of sulfate. Based on both the infrared examination and the lack of reactivity in the sulfate analysis, the acidic glycolipid fraction did not appear to contain nativesulfate or ester groups. After the upper phase fractionwas combined with the appreciable amount of acidic glycolipid remaining inthe treated Folch lower phase and following purification of this pool by Iatrobeads column chromatography, TLC of the acidic glycolipid fraction, shown in Fig. 1, revealed two major components, glycolipids X and Y. Yields of these components were 11 and 6 mg, respectively, by weighing. Reactivity of Glycolipids X and Y with Standard Chromatographic Spray Reagents-The reactivity of the two acidic glycolipids to a number of chromatographic spray reagentsis shown in Table I together with uronic acid content and RF values. The reaction of glycolipid X was positive to orcinolHzS04 and b-vapor andnegative to the ninhydrin and phosphate spray reagents. In contrast, the positive reaction of glycolipid Y with all these reagents suggested the presence of an aminoalkyl group linked through a phosphate bond to a sugar moiety. This presumption was strengthened by the close similarity between the present results and the observation made in our earlier study of Corbicula glycolipid 16 ; . HF Treatment of Glycolipid Y-Glycolipid Y was therefore and bumetanide. HIV-positive patients' insistence on confidentiality results in a negative attitude, on the part of informants, towards such a patient. Informants regard such patients as being uncaring persons. As a result of this, informants themselves become uncaring caregivers to those patients and may even neglect their patients. One informant explained: "A pregnant woman did not want to divulge her positive HIV-status to the husband and I thought that woman was not good. I sometimes neglect HIV AIDS patients because of being uncertain of my safety when giving them care". "There was a HIV-positive patient admitted in the ward. The lady wouldn't divulge her positive status to the husband and she continued having a sexual relationship with the husband without using a condom. It was obvious because she was pregnant. These HIV patients do not have feelings for their partners. They prefer to spread it. The husband never knew that the wife was HIV-positive. I think you can as well neglect them because they also don't care for others!

The synthesis of cGMP is catalyzed by guanylyl cyclase GC ; , the enzyme which converts GTP to cGMP. There are two main forms of guanylyl cyclase, membrane-bound GC and soluble GC47. Membrane-bound GC is activated by hormones such as the natriuretic factors48, while nitric oxide and nitrosamines typically stimulate cGMP synthesis in soluble GC49.
